Form a Chapter

ULTRA Teen Choice (UTC) Chapters carry out the mission and purpose of ULTRA Teen Choice. UTC Chapters enable organizations, schools, churches or other groups to provide education, peer counseling and accountability relationships for their youth and young adults. To apply to become a Chapter fill out the Chapter application and return it to ULTRA Teen Choice by email, fax or mail. We will contact you to schedule an in person or telephone consultation to review the materials and process, as well as answer any questions.

Financial assistance may be available through our Partnership with Servants Without Borders. Please read the message below from Servants Without Borders Abstinence Leadership project director Lynura Jones and submit your application by email or mail to:

Dear Churches and/or Community Organizations,

On behalf of Servant's Without Borders (SWB), I would like to invite you to become a part of the world-wide Abstinence Revolution by becoming a regional chapter of the Ultra Teen Choice program. This program includes information and activities that promote HIV/AIDS prevention and behavioral change through abstinence. Becoming a chapter in this program will inspire the youth in your group to remain abstinent until marriage and will connect them with other youth in the region who have decided to do the same.

Because we know funding can be scarce, SWB is working to offer the chapter start up kit and training to eligible groups free of charge. To apply for funding you must complete the chapter application and submit it to ULTRA Teen Choice either via email: or mail applications to ULTRA Teen Choice, PO Box 48608, Washington, DC 20002.
